What Is Medical Coding?

Medical coding is translating a patient visit, including the diagnosis, procedures and supplies, into standard codes, mainly ICD-10 for diagnoses, CPT for procedures and HCPCS for other services and supplies.

The codes tell the insurer what was wrong, what was done and what was used, and they determine how the claim is priced and paid. Accuracy matters: wrong or unsupported codes cause denials and compliance risk.

Coders work from the clinician’s documentation, so clear notes at the point of care make coding faster and more accurate.
